Switching is simpler than most people expect
Your Support at Home approval belongs to you, not to your provider. If you’d like a change, whether because of price, flexibility or how the service feels, you can choose another registered provider. Guardian Home Support serves Perth Metro, Rockingham and Mandurah.
How to switch, step by step
Talk to us first
Ask your questions and tell us what you’re approved for. There’s no obligation, and you don’t have to tell your current provider yet.
Check our prices and what we can deliver
Our weekday rates are on our Services page. All your Support at Home services need to come from one provider, so we’ll confirm we can deliver everything you’re approved for.
Give us your referral code
Once you’ve decided, call My Aged Care on 1800 200 422 to reactivate your referral code, then give the code to us. It lets us view your support plan and accept your referral. When we accept it, your current provider is notified.
Let your current provider know
Tell your current provider the day you’d like your services with them to finish. We’ll agree your start date with us so it falls on or after that end date, with no gap in your care. Your service agreement with them may include conditions about ending it, so it’s worth a quick check.
Sign a service agreement with us
This sets out your services, who provides them and what they cost. Services can’t start until it’s signed, and we’ll go through it with you in plain English.

Common questions
Will I lose my funding if I switch?
No. Your Support at Home approval stays with you. You reactivate your referral code with My Aged Care and sign a new service agreement with your new provider.
What happens to unspent funds when I move?
According to My Aged Care, your previous provider has 60 days from your exit date to finalise any outstanding invoices, and any unspent contributions you paid are repaid to you within 70 days after your services stop. Your new provider may not have accurate budget details until about 28 days after you start, so we’ll talk through your remaining quarterly budget with you early.
Do I pay a fee to switch providers?
No. The Department of Health, Disability and Ageing states that you can change providers at any time and providers cannot charge an entry fee or an exit fee.
How long does the switch take?
It depends on how quickly your referral code is reactivated and we’ve agreed dates with you. Your start date with us needs to be on or after the end date you agree with your current provider, so there is no gap in your care. Call us and we’ll talk through a realistic timeline for your situation.
Do I have to tell my current provider before I switch?
You do need to tell your current provider the day you plan to stop receiving services from them. My Aged Care suggests checking your service agreement for any conditions about ending it. If you’re unhappy with your current service, you can also talk to them first, and a free advocate through the Aged Care Advocacy Line on 1800 700 600 can help with that conversation.
Can I keep the same days and times?
We’ll try to match your current routine where we can. Tell us what matters most and we’ll say what’s possible.
